CONNECTING WITH US

We all want to make seeing a primary care provider simple and easy. That’s why PACE-RI handles all the details. Our team works tirelessly to provide full-service medical care when you need it most. That includes transportation to and from medical appointments. This door-to-door service covers advanced primary care visits as well as appointments with outside specialists. On top of that, we also deliver prescriptions right to your door! Keeping yourself healthy has never been easier thanks to the comprehensive care from PACE-RI.

Advanced Primary Care to Meet Every Need

The PACE program is about more than just providing seniors with health care. The mission of our organization is to keep participants safe at home and as independent as possible.

It all starts with the first visit to one of our primary care specialists. In that assessment, our team will talk about your past medical history, your current health status, and your goals for the future. Then we take a broader look at the options available to you for care.

To do that, we utilize a team of experts from a variety of areas. Together, these professionals create a treatment plan for the participant and ensure that their health outcomes are being met every step of the way. These professionals are part of an interdisciplinary team that oversees all aspects of a participant’s care. It combines experts from disciplines such as:

  • Nursing
  • Social Work
  • Primary Care
  • Transportation
  • Nutrition

We Work with Medicare and Medicaid

Many older Rhode Islanders live on a fixed income, making it difficult to afford needed medical care like prescriptions or dental work. For seniors who qualify for Medicare and/or Medicaid, PACE-RI provides primary care services as part of a holistic approach to keeping older adults safe and well at home. This program also includes:

  • Day Center Services
  • Rehabilitation
  • Behavioral Health
  • Home Care
  • Prescriptions
  • Life Enrichment
  • And more!

PACE-RI brings together all these services under one roof, making it easy for seniors on Medicare and Medicaid to get the help they need without breaking the bank. As a non-profit health plan, we strive to bring top-notch medical care to our participants both at home and in our health center. Maybe that’s why nearly 90% of PACE-RI participants would recommend PACE to a close friend or relative!

A Medical Center for Primary Care and More

The PACE program is a comprehensive way to bring primary care providers and other specialists to older adults who need them. Our mission to keep folks living at home and out of long-term facilities means our health center must be suited to meet the needs of our participants. This means our health centers are fully accessible, with features like bariatric chairs and grab bars to ensure ease of access and safety in the facilities.

Our medical center is equipped to handle primary care visits and routine care and can accommodate specialists of all types. Whether you need a podiatrist, physical therapist, or optometrist, the PACE health center will have the facilities and personnel needed to get you fit. That even includes bringing outside specialists into the PACE Health Center for services that elderly Rhode Islanders might struggle to find.

With so many options today, it can be difficult to find the right partners for your healthcare. That’s why PACE-RI is here. We take the stress and confusion out of health care and provide the expertise you need to stay healthy. From mental health and primary care to rehabilitation and rides in Rhode Island, the team at PACE-RI is here to guide you to a happier and healthier tomorrow.
